The Home and Community Based Services for the Elderly, Blind and Disabled (HCBS EBD) are services provided in the home and community setting to individuals who are eligible for Medicaid reimbursements for long term care needs. These individuals would otherwise require nursing facility care without the provision of HCBS-EBD services, with the cost for care no more than the cost of the nursing home.

Eligibility of HCBS services are determined by the local Department of Human Services in Huerfano County. Once an individual is deemed eligible, the referral will be made to our office for staffing.

Clients can be authorized by the Department of Human Services for either relative or non-relative care assistance. Authorization is given for a specific length of time and then reauthorized as needed by the Department of Human Services. Depending on the service authorized, relative care givers other than a spouse, may be considered and employed by the agency if they meet employment requirements through this Medicaid Program.

Non-medical transportation which enabling the client to gain personal physical access to non-medical community services and resources can be authorized by the Department of Human Services.

Specific adaption or installations in an eligible client's home setting are allowed through the Medicaid HCBS-EBD benefit. These adaptions or installations must be necessary to ensure health, welfare and safety of the individual. This primarily includes wheelchair ramps and bathroom modifications.

Clientele who do not qualify for Medicaid assistance are still entitled to care within our array of services. We offer personal care providers and homemakers through our private pay program. These providers offer the same quality of care, the only difference is that the determination of service hours is done with the client and their family members along with our Case Management Staff. Programs can be developed to meet whatever needs arise whether it is a few hours weekly to many hours daily. In some instances, we will work with other skilled programs to offer round the clock care for clientele.

Clients on the Private Pay Program can access grant funding when available if they are over age 60, a US Citizen, and have not previously accessed the grant for care. Typically, grant assistance is for a shorter period of time which allows the client time to plan for future needs, to work through the process of long term care or for the purpose of recovery from illness or surgery.

Our Case Management team will work with the client and their family members to assist with establishing and billing third party long term care insurances. Typically, these insurance programs require that a client complete an eligibility period before the insurance can be activated to cover the cost of care. Staff will assist with supplying documentation to the insurance company in order to establish this care.

The South Central Council of Government also works closely with the Department of Veteran's Affairs on providing homecare services to eligible veterans. Determination of these homemaker services is done with the Department of Veteran's Affairs and SCCOG will handle all billing directly with the Veteran's Affairs Department.
