What We Offer
Unlike skilled home health care, which requires a physician’s order and clinical oversight, our services are flexible, and client centered.
Our services include:
- Personal care assistance
- Daily living support
- Companionship and social interaction
- Customized care planning
Each client receives a home assessment by our Case Management team, followed by a tailored care plan developed in collaboration with the client and their family.

Payment & Funding Options
We work with a variety of funding sources to make care accessible:
- Medicaid Home and Community-Based Services (HCBS)

- Grant assistance (when available through the Huerfano Las Animas Area Agency on Aging)
