Care Services Program

The South Central Council of Governments (SCCOG) Care Services Program is licensed by the Colorado Department of Public Health and Environment to provide non-medical, in-home support in:

  • Huerfano County
  • Las Animas County
  • Limited areas of Southern Pueblo County

Our goal is to help individuals remain safely and comfortably in their homes for as long as possible, with personalized care that promotes independence and well-being.

 

What We Offer

Unlike skilled home health care, which requires a physician’s order and clinical oversight, our services are flexible, and client centered.

Our services include:

  • Personal care assistance
  • Daily living support
  • Companionship and social interaction
  • Customized care planning

Each client receives a home assessment by our Case Management team, followed by a tailored care plan developed in collaboration with the client and their family.

Payment & Funding Options

We work with a variety of funding sources to make care accessible:

  • Medicaid Home and Community-Based Services (HCBS)
  • Private Pay
  • Grant assistance (when available through the Huerfano Las Animas Area Agency on Aging)

We operate two administrative offices to serve our region:

Our provider staff are available throughout both counties, including the most remote communities.
