SERVICES AVAILABLE THROUGH COLORADO HEALTH FIRST (COLORADO MEDICAID)
HOME COMMUNITY BASED SERVICES
The Home and Community Based Services (HCBS) program offers non-skilled personal care, homemaker services, and In-home support programs through the Elderly Blind and Disabled Waiver (EBD) and through the Community Mental Health Supports Waiver (CMHS). Eligibility is determined by the Huerfano Department of Social Services and then the referral for care is directed to our agency. Clients can be authorized for either relative or non-relative care assistance. Each authorization is for a specific length of time (typically one year) and then reauthorized as necessary. Depending on the service authorized, relative care givers may be considered and employed by the agency if they meet employments requirements through the Medicaid Program and our agency.
PRIVATE PAY PROGRAM
Clientele that do not qualify for Medicaid assistance are still entitled to care within our array of services. We offer personal care providers and homemakers through our private pay program. These providers offer the same qualify of care, the only difference is that the determination of service hours is done with the client and their family members along with our Case Management staff. Programs can be developed to meet whatever needs arise whether it is a few hours weekly to several hours daily. When grant funding is available, Case Management will help the client to apply for financial assistance. Older American grant funding is only available to clients over the age of 60 years, a U.S. Citizen, and meeting the qualification of the homecare or personal care criteria.
Our team will work with the client and their family members to assist with billing third party long term care insurance. Typically, these insurance programs require a client to complete an eligibility period before the insurance can be activated to cover the cost of the care. Staff will assist with supplying documentation to the insurance company to establish this care.
