Advocacy Programs / Care Services

QUALITY ASSURANCE TEAM 
The South Central Council of Governments Care Services Program strives to provide a quality based care service program assuring client care and safety as required by regulations in Chapter II, 6 CCR 1011-1, Section 3 of the Colorado Department of Public Health and Environment.  The Agency is committed to the development and implementation of a Quality Management Program and will review an act or omission that is below the agency’s standard of care or that has caused or could cause injury to a client or violates client rights.  
        
The Agency’s plan will evaluate the quality of client care and safety to include the following information:

  1. A general description of the types of cases, problems, or risks that will be reviewed and criteria for identifying potential risks;
  2. The personnel or committee responsible for coordinating quality management activities and the means of reporting to the director and governing body;
  3. A description of the method for systematically reporting information to a person designated by the facility and within a prescribed time;
  4. A description of the method for investigating and analyzing the frequency and causes of individual problems and patterns of problems;
  5. A description of taking corrective action to address the problems, including prevention and minimizing the problems and risks;
  6. A description of the method for the follow-up of corrective action to determine the effectiveness of such actions;
  7. A description of the method for coordinating pertinent case, problem, or risk review information with other applicable quality assurance and/or risk management activities;
  8. Documentation of required quality management activities including cases, problems, or risks identified for review, finding of investigations, and any actions taken to address problems or risks; and
  9. A schedule for pain implementation not to exceed 90 days after the date the facility receives written notice of the Department's approval of the plan.  

INTERDISCIPLINARY MEETINGS
The Huerfano Care Service Program meets regularly with local home health programs to address care planning, coordination and service delivery thus avoiding any duplication of services.  The coordination of care allows our program to assist in the development and implementation of services to best address client health concerns. 


ADULT PROTECTION TEAM
The Care Service Director and other Case Management staff participate in the Huerfano Adult Protection Team.  This team’s mission is to address those individuals at risk in our community by identifying their specific needs, problem solving with representatives of other agencies, and when possible providing services with the goal of improving the living conditions of those identified.
